WHAT TO TAKE TO THE BMV

To get your permit.

To obtain your Driver Education Learner Permit, you will need:

  • To be at least 15 years of age at time of enrollment.

  • Students will be required to pass a written test and present a valid CDE form to receive a driver education permit. Students may enroll in driver education, and may complete any or all of the classroom portion of the course without having to pass a written test. However, they must pass a written test before a permit will be issued. They will NOT be required to take the written test again when they apply for their license.

  • To provide proof of identity and lawful status (original birth certificate), your social security card, and two documents to establish Indiana residency (bank statement, ID card, etc.) Log on to www.bmvexpress.in.gov for a current ID list.

  • To successfully pass the standard vision screening test.

If less than 18 years of age, have a parent/guardian sign the financial responsibility agreement.

To get your Probationary Driver's Licence.

To obtain your Probationary Driver's Licence you will need:

  • Learner's Permit

  • 50 Hour Drive Log Print one using this link. (Fill out in blue or black ink)

  • Certificate of completion for the the 30 hour online course and the 6 hour behind the wheel course.
